Puppy Sitting Form

This form needs to be given to a puppysitter each time you leave your puppy with another member of the group. Your leader should also be informed each time your pup goes to another raiser. Make a copy of the completed form for yourself and another copy to be included with your Monthly Progress Report.

Puppy Travel Form

Travel with your puppy must always be approved in advance. Fill out this form and submit it to your leader as far in advance as possible. Our advisor, Tami Shankle, will be sent the form with the leader's signature, for final approval. That is one reason for submitting the form well in advance of your planned trip.
